Each client will be responsible for full payment of fee or co-payment at the beginning of each session. I accept most methods of payment and will be happy to bill a HSA as well.
Private Pay Rates:
$250 for Initial Session (60min)
$175 per individual therapy session in the office or via telehealth (45min)
$200 per individual therapy session in the office or via telehealth (60min)
$225 Family or Couples therapy (60min)
$225 for home-based perinatal sessions (for clients who choose this after they have a baby, or who are on bedrest)
I offer sliding scale as well.
Insurance
I accept some insurance plans. Each client's behavioral health plan varies.
Services may be covered in full or in part by your health insurance or employee benefit plan. Please contact your health plan to check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
- Is Carolyn M. Chapman, LICSW an in-network or out-of-network provider?
- Does my plan provide out of network benefits?
If I am an out of network provider, and your plan provides out of network benefits, you will be responsible for payment of the full fee at the time of your visit. I can provide you with the appropriate paperwork that you can submit to your insurance for reimbursement.
Reduced Fee
Reduced fee services are available on a limited basis.
I save your appointment for you and no one else. If you do not show up for your scheduled therapy appointment, or you have not notified me of cancellation at least 24 hours in advance, you will be required to pay the $100 no-show fee . Insurance does not pay for a missed session. Exeptions include cancellations due to hazardous driving conditions or illness.
